Please fill out the following form to apply for a course at the Seaman's Institute.

I understand that payment must be made in full ten days prior to the start of the course(s), that tuition deposits are not refundable after that ten day period, and that if the School cancels the course I will receive a full refund. I understand there is a dress code, an ethics code, an attendance requirement, and other regulations which if violated may result in my immediate removal from the School without any refund of tuition or fees. I further understand that there is a potential for injury that is inherent with some of the activities of the School, that I may opt-out of participating in such activities, and that I hold the School harmless from any personal injury claims.
